Subject: Extended Health Rate Changes – Effective January 1, 2014

Your extended health benefits plan assists in reimbursing for specified medical services and supplies while your dental care plan assists in reimbursing for dental care costs for services such as exams, cleanings, x-rays, fillings, crowns, bridges, and orthodontia.

To ensure that the rates are at the appropriate levels, the claims experience of both plans was evaluated. As a result of increased claims usage, it was necessary to initiate increases to the Extended Health rates (approximately 2% increase). However, no changes are required to the Dental Care rates.

Since the premium rates for your Extended Health and Dental Care plan are 50% employee / 50% employer paid, your portion of the dental care rate will change effective January 1, 2014, as follows, and will be reflected on your December 31, 2013 pay advice (premiums are taken one month in advance).
