Income Replacement Plan/Disability Benefit Plan Application Process

If after reviewing your employee group’s Income Replacement Plan/Disability Benefit Plan, you determine that you meet the eligibility criteria and you wish to apply for benefits, please proceed with the following steps approximately three months before the end of your elimination period:

  1. Download, print off and complete the Sunlife Disability Claim Form (Member). (Please note that the Disability Claim Form replaces the old Member Statement Form and Physician Statement Form.)
  2. Contact the Disability Benefits Claims Assistant by emailing chelsea.stout@ubc.ca or by phoning 604-822-8696 to initiate form processing by your department.
  3. Return completed documents, including relevant medical reports to support your claim, approximately two months before the end of your elimination period along with a copy of either your driver’s license, birth certificate or passport (for proof of age) to the Disability Benefits Claims Assistant at the below address:Disability Benefits Claims Assistant

The Disability Benefits Claims Assistant will forward your application documents directly to Sun Life Assurance Company of Canada. Alternatively, you can submit the completed documents directly to Sun Life Assurance Company of Canada at the address provided on the application forms.

Health Promotion Programs (HPP) at UBC only collects, uses and discloses personal information, including employee health information, for the purposes stated. The Disability Benefits Claims Assistant provides this service for the applicants’ convenience.
