Please inform Amy Kao of your changes through telephone, email, fax, or send a letter. We will make the change and send you a confirmation letter for your records for all of the above except personal information and bank account.
For changes to your banking information regarding your pre-authorized debit for your premiums, we will require a void/sample cheque from the new bank account or a letter from your bank with the new banking details (bank#, transit # and account#).
Your change request for banking information and benefits coverage must be received by the 15th of the month to be effective on the 1st of the following month. For example, if you want a change to be effective April 1st, you must let Amy Kao know about your request by March 15th. Otherwise your change will start May 1st.
In the event of the death of the primary registrant, as the surviving dependent, you will need to contact Amy Kao to discuss the continuation of your benefits coverage, and make the necessary changes to your enrolment.
