For quicker processing and payment, consider submitting your UBC Dental or UBC Extended Health claims online. You can also register to have your reimbursement deposited directly into your bank account. Both are easy to do and kinder to the environment. Login to the Sun Life Members website to file your e-claim, or visit the Claims section of the UBC Benefits website for step-by-step instructions.

You can enrol for your benefits online by using UBC Benefits Sign-on.

For those without access to a computer, or if you are required to submit original receipts, you can file a paper claim by clicking on the appropriate form below.

Please note: Only vision care and paramedical claims may be submitted online.  Prescription drug claims that have not been processed using your pay-direct drug card may also be submitted online.

Non-UBC Management & Professional Staff (i.e. Paymaster employers) – please use the regular (not combined HSA) claim form.

Medical Services Plan (MSP)

Note: Enrolment in MSP is mandatory for all residents of British Columbia. However, enrolment through UBC’s group plan is not necessary if you have MSP coverage through another benefits plan.

Please note that the Disability Claim Form replaces the old Member Statement Form and Physician Statement Form.

To enrol in the IRP: Download and complete the following form applicable to your UBC employee group.

• Faculty form (IRP): Faculty, Academic Executives, Administrative Executives considered High Earners, Staff High Earners
• Staff form (IRP): CUPE 116

Extended Health and Dental claims forms can also be downloaded from the Sun Life member website, using your access ID and password.

Payroll Forms

Faculty and staff can update and view their personal and payroll information quickly and securely through the UBC Faculty & Staff Self-Service web portal. Simply access the portal using your Campus Wide Login (CWL).

For those without access to a computer, you can submit a paper payroll form for the following:
