During your time at UBC, you may need to makes changes to your benefits plan: Change your personal information, change or add beneficiaries and dependents, or choose to change the amount of coverage you carry.
First determine the type of change you need to make, then complete the appropriate change forms and submit them to Payroll in Financial Services (see addresses below).
Personal changes – Adding or removing dependents, and changing beneficiaries for insurance and pensions are just some of the changes you may choose to make, depending on changes in your life such as:
In the event of your own death, if you are the primary holder of the UBC benefits plan, your spouse and dependents may be eligible for benefits under UBC’s Retirement and Survivor Benefits Plan.
Changes in coverage – For optional benefits, such as Optional Life or Optional AD&D insurance, you can change the amount of coverage you carry, as long as you don’t exceed the maximum allowed on your plan. Changes in insurance coverage must be in blocks of $25,000.
To increase your insurance coverage, you must be actively at work, and you will be asked to submit a health questionnaire to Sun Life.
Some life changes may affect who will be your Pension beneficiary. Contact the Pension Administration Office at 604-822-8100 for more information.
Contact the appropriate Benefits Administrator in Payroll Services for changes to Benefits other than Pensions, or download the form you need from the Benefits Forms page. Once complete, send the form to Financial Services.
